| In May 2019, the University of Washington was in the process of removing one of its CsCl irradiators. During the removal, the source was compromised resulting in measurable contamination of several buildings and grounds and several people. The incident made national news. This PEP will provide an event timeline, the status of the recovery and lessons learned. |
